While I can't completely rule out complete incompetence on the part of the Goatwings Airline pilot, Iran has a history of playing chicken with our air defenses. They have yet to win a single round. Some folks just don't learn.

Iran also has a history of lying like hell. Don't put it past them that the pilot maneuvered on purpose, or that passengers were hit (and told to play along) to make it look like they got injured by the maneuver. An asslifter showing what looks like blood coming down from a head wound? So what, Hollywood all the time makes it look like someone was shot, exploded, etc.
